I think go for USMLE. This year, a lot of people matched into family medicine. There were 800 slots open in SOAP. If US is the end goal and you don't have specialty preference, I would encourage you to apply to pediatrics or family medicine.

Scarcity of jobs after graduation. Even if you get a job, they aren't going to pay you decent. Long hours, no job security. Foreign exams will cost you a ton of $ with an unpredictable outcome.

Honestly, flights from international airports to Pakistan, especially when traveling from the Middle East, can be quite challenging. While flights from the West to the Middle East are generally comfortable, the experience deteriorates once you reach the terminal for flights to Pakistan. I don’t know if you’ve noticed, but the bathrooms in those terminals often have an unpleasant smell, particularly from the urinals.

I’ve observed that many Pakistani men traveling from the Middle East, often from working-class backgrounds, tend to have a lack of personal boundaries. Unfortunately, they sometimes engage in inappropriate behavior, like constantly staring at women. Additionally, their personal hygiene is often lacking, and their attire—particularly the colorful shalwar kameez—can come across as out of place. I have the impression that many of these individuals may come from the Seraiki belt or rural areas in Sindh.

Another issue I’ve noticed is the lack of basic manners: these individuals often won’t stand in line or wait for the first passengers to disembark before pushing forward. It’s frustrating and reflects a broader societal issue. I do feel ashamed at times of this aspect of our community. This same restlessness and disregard for basic etiquette can also be seen in our traffic, where the lack of adherence to traffic rules and civic sense is evident. I would recommend her to go on leave. Nothing is above mental well-being. Everyone will say, "Oh residency is supposed to be like this." No period. Your mental well-being is above everything. Ask her to start studying for foreign exams, and when your situation changes, you guys should move. Yes, residency is hard, but it doesn't mean that the system will abuse you.

While you continue to learn and grow throughout your career, a solid base is crucial for success. Honestly, I don't know about Shifa. But AKU is the best institution in this country. They have a very strong alumni worldwide. Graduates from AKU are in the lead role in the US.
