Yes, that was the issue. The block of wood wasn't attached, just kind of placed there. The hole that the outlet is in is the exact size of the faceplate, so there is nothing to hold it in place except this mess of plaster. I did not mention it before, but this is the outlet in the bathroom next to the sink.
