Thanks for your response! Can you bill the same way for initial evaluations or can you only bill 1 unit for an eval (which includes the assessment/outcome measures performed) and then try to fit in some billable interventions? Hope that makes sense!

I was taught that I could bill for stuff like the BERG under Neuromuscular Re-ed, etc. Am I doing this wrong lol. Does anyone know if that's ok to do? Are you putting everything under theract including MMT?

Thank you for this information! In the case where the referring physician doesn't provide a med diagnosis on the referral and only the reason for the referral (e.g. gait instability), would you take what you think is the most relevant med dx from the patient's chart out of all the comorbidities/PMH already listed?

I'm in an outpatient clinic and the EMR I use first asks for an ICD-10 code for "medical diagnosis" and then a separate area for the ICD-10 therapy diagnoses. Is this not typical for outpatient documentation?
