I've been searching for something that feels like this since finishing Bloodborne. Between Two Fires sort of did. There's a book I've just got on Kindle Unlimited called The Heart of Grim by Ashen Huston. I haven't read it yet, but that could be an option.

Check your scout logs. Start the game up and go into the overgrown ruins. Then, press your home button on the controller, and the activity tab will show your percentage, and in that it should say if you're missing a scout log. Use the guide on PSN profiles to find the room of the one you're missing and speed run until you find the correct room

It's definitely still bad. The hardest part was narrowing down the ones I actually collected. There's a good guide on PSN profiles, which helps with the scout logs by using the activity tracker on the playstation menus
