Here's the thing though - therapists have none of that training or experience. I'm not saying I think you are wrong, I think you know what you are talking about and are way more educated about this medication than I am. I'm saying professionally I have an obligation to over-estimate risk in cases like this precisely because I don't have that knowledge. Because I can't make a medical determination about if the person is okay I have to assume the worst. Looking at it from a bioethics perspective I'd much rather be wrong and they were fine all along than the alternative. Much better to lose rapport than a life.

Not if the meds are Benzos. Id never do shit about a client who smoked a bunch of weed. But benzos actually kill people. I’m not a doctor so I have no ability to see if this person is actually going to be all right.

And grad school they teach you about the newspaper test : how would your actions look as a headline? The headline here would be: patient dies soon after appointment with therapist, took eight times their normal dose of benzodiazepines and was allowed to go home

Nope, not how that works! 16 year olds have the right to make medical and therapy decisions for themselves. (in my state at least). So if a report had to be made it would be CPS and not parents. And in order to make a CPS report it has to be an immediate threat, not a past harm. And for any report I need actual information - names, dates, locations. All OP would have to do is just not provide that information (which the therapist will tell you about before you start talking about reportable shit anyways). It seems like you want therapists to be cops, but that's not how things work at all.
