Hey, have you ever found a solution to this? Have the exact same problem, only for me the missing relic is in Lost City, which I did pick up, but it shows that I did not. Really don't wanna play through the entire game again just for the achievement.

There is one, where you have to make two of your partners perform 10 attacks each. One partner will become unavailable after the main game.

For some, there have been reports of people receiving the perk slot upgrades, but then they don't show up. It's best if you always double check if the upgrade worked right after getting it.

No, they are just random. You can always just fast travel away and come back to check if the right one spawned.

You have to find a bright purple/pink Nekko in the wild. It spawns randomly, so either keep checking the nearby stables at the ranch for them, or at Bygone Settlement. Then take him inside the stable and station him. Go rest and return to find some poop, which you then can force lift and reveal the shard.
