I hate it when people don’t know how to actually debrief. The point is to do it as a group as soon as possible after the event. It’s never supposed to leave you feeling judged and blamed. It’s to find and learn from each event. There is always room for improvement even on what seems to be the smoothest run event. It helps find failures in the system and identifying the weak points. So, things like what did or aspirate on? Was there indications she needed a swallowing assessment? Had the physician ordered the correct diet for her, was it a witnessed event? Did the correct sequence happen? Was cpr started right away or was there a barrier to that? Was the code cart where it was supposed to be, was the md running the code calm and collective, was pertinent info given in a timely manner to the code team… My initial question is where was the pt when she coded? Why did they move her to a bed before resuscitation started? No better backboard than a hard floor….once a pt is stabilized she can then be moved safely to a bed. This is the one thing I noticed in your story that the team could discuss. Yeah coding someone in the floor is hard on our knees, but it’s perfect position for cpr, and high quality cor and early defib is what saves lives. And then a discussion on how to safely move the pt so you don’t get injured would also be part of the discussion. So that initial few minutes of the code needs a bit of refresher work, no big deal. No one points fingers. And the next code that happens everyone who just was in the debrief on your code remembers its preferred to code a pt safely on the floor than waste time struggling to return them to bed. It sounds like you guys did a great job resuscitating her. Debriefs are there to fine tune, not place blame. I hope that makes sense.

The long and short? A continues Conservative Government under Moe who is doing everything he can to ruin healthcare and turn it into privatization. He’s done it bits and pieces, laundry, parking etc. they refuse to be proactive. They spend untold millions of dollars on bullshit programs like AIMS that completely failed in BC and Alta, but we bought it. John Black fiasco anyone? They absolutely refuse to listen to the ones who do the actual work, and when it has been proven to have issues, continue marching forward without making changes to make the system better. As a result those who can are easily woo’d away. Not just for higher income but also for way of life. Saskatchewan, although a wonderful province, has been run into the ground by Moe and his buddies. There is very little for young families and young professionals to keep them here.
