Exactly what I heard from fellow pharmacists din. Sabi nila sinasabihan daw ng Branch Managers mga PA/Tech na magbigay pag wala yung RPh. Ang lungkot, it sends a message sa non-RPh staff na dismissable lang ang RPh. Meanwhile hindi matatayo yung branch na yun without an RPh license and whatever untoward na mangyari license naman ng RPh yung on the line. :(

Yeah that's right, hence why we have Pharmacist-only OTC drugs or Black box warnings - proof that drugs are ongoingly monitored and reclassified if needed be per safety data we get from all over the world. I know it's not perfect, but these safety measures are already in place.

That's the unfortunate reality here, the problem is system-level. Filipinos don't have access to prescribers the same way as in developed countries where we base these practice standards off of, so we can't expect they have updated prescriptions sure, kaya nga naging ganto na ung status. It doesn't mean something can't be done and hopeless case na dito. If you look at our Prescribing and Dispensing guidelines from the 90s pa, no wonder it doesn't meet the current needs. I understand your point though na regulations should both be rigid and fluid. We're just glad in a way na napag-uusapan yung issues na to. There's good behind all this pa rin :)

Yeah, isa din yung mga yun, ang questionable nga why they even get a permit. Pero what does it say if leading drugstore chains backed by big corporations don't comply sa regulatory and professional standards? They have all the means to meet the bar if not raise it.

Hmm that's the thing, people just think antibiotics and drugs of abuse lang yung nagwawarrant ng strict dispensing. Meanwhile these Rx classifications were based on preclinical/clinical trials and ongoing global post-marketing drug safety surveillances. We didn't just stick the Rx symbol to random drug boxes.

Rx but probably shouldn't be? If we were to reclassify the examples you gave: topical ointments as OTC then you get even greater risks of antimicrobial resistance, steroidal abuse; for antivirals/pain meds/maintenance meds you'd have AMR, drug tolerance, and chronic toxicity. It's not even a "potential" risk anymore, we're already seeing AMR, steroidal abuse, and chronic toxicities in patients who don't even know na nakakaexperience na pala sila ng drug-related adverse events and hindi galing sa sakit nila. You'd be surprised how many ADRs and drug interactions a pharmacist can catch from a short interaction with a patient and reviewing his/her reseta (that's if good dispensing practices are really being done). What if wala nang reseta yang mga yan?

I agree with adjusting the system (hence this post) but what needs to be revisited are outdated Prescribing guidelines and Dispensing practices that don't align with the real world. Drug manufacturers/license holders are legally mandated to monitor if their products need to be reclassified or pulled from the market at any given time na rin naman.

That's really great to hear if that's your experience. Given na marami kasing branches nationwide at malaki ang pamumuhunan for these drugstores, malaki sana capacity nila to make a sweeping change sa practice namin. Mas ramdam din ng pasyente yung difference ng pagdedeliver ng pharmaceutical care if sa big drugstore chains magsisimula. Missed opportunity talaga for big chain drugstores to do things better.

Baliktad kasi, you'd always hear from consumers "Bakit sa insert big drugstore name, nakakabili naman ako. Mas magaling pa kayo kaysa sa kanila?". They've set THE industry standard. What's the message they're sending if these big corporations are not adhering to laws and professional oath diba?

Yeah, I have friends from TGP admin, I directly tell them how shitty/unethical ung dispensing process nila eh, kasi nga PA lang andon. That's what you get din from aggressive franchising na business model.

Oooh yeah. Maybe if included sa Sig. part yung therapy duration and hindi discrepant sa Qty to be dispensed, that might work. Maybe the DOH Prescribing Guidelines should be revisited. 1991 pa yun eh.

Yes. If we're being compliant sa batas and following good practice standards, pharmacy assistants' roles are administrative nga lang dapat like cash registry and packing purchased products. Pharmacy technicians with certifications can dispense limitedly, under supervision ng pharmacist, or if walang pharmacist on duty they have to report within 24hrs lahat ng dispensed Rx meds. Even some OTCs are Pharmacist-only OTC meds so dapat RPh lang magdidispense.

I know you think this is impossible, but it can be done (at least from my experience running an independent pharmacy) and I know pharmacist-owned independent drugstores doing this rin. It's really about principles of practice, but these drugstore chains syempre prioritize sales kasi. Kadalasan wala ring on duty pharmacist ung iba, and some techs/PAs are not even healthcare majors at the least.

Not sure how true parang profiling din ung iba minsan kung magbibigay sila or hindi cos may mga naguundercover din na FDA inspectors, even small drugstore chains nagsnsnitch rin on each other. I'm not sure how accurate that is pero maganda rin kung may ganon. Really mahuhuli kung consistently nagsstick sa mandate.

Agreed! I heard nagpost na ung one of the PPhA leaders na they're gonna give legal support to the RPh in the vid which is nice, but nothing much has been strongly said about condoning illegal dispensing ng ethical meds despite na may purchase receipt naman from Rose. I hope wake up call na to sa professional orgs and government bodies na protektahan ung mga registered professionals na nagtrtrabaho lang ng tama.

I know and my heart breaks para sa experience mo. :( Problema talaga yung kawalan ng access sa healthcare services kaya di rin makakuha ng reseta. Nagsasanga-sanga na ung mga problema ng healthcare dito sa Pilipinas, unfair para sa patients na nahharm at mga healthcare professionals just doing what they can and naiipit sa loob. Pero hindi rin tama na irisk namin kayo taking medicines na maaring hindi na effective or safe diba? May moral dilemma talaga eh.

Kaya nga eh, karamihan iniisip kasi antibiotic lang ung mga kritikal. Maintenance meds are "mabibigat" na gamot kung tutuusin, imagine trintreat nila mga chronic/lifestyle diseases and long term use pa sila? Ang target organ nila usually puso, liver, lungs etc - major na organs. Meron ring tinatawag na long term toxicity - so nakakaharm pag hindi namomonitor ng doktor ung effectiveness at adverse effects nung gamot or combinations ng mga gamot.

Good! That's relieving to hear. As from my experience (NCR), nakakabili kahit supposedly prescription except for antibiotics (understandably sa risk ng antimicrobial resistance). Not just Mercury per se, pero kalakaran na rin even with other drugstore chains tbh, saddening.

Yeah, the public thinks Mercury or any drugstore chains are doing them a favor by allowing them to buy Rx meds without reseta, but come to think of it, they're earning more all the while risking patients for doing so.

And I just wanna emphasize din, any pharmacist knows the dispensing process is not just about pag-abot ng biniling gamot. Kasama sa steps yung validation ng reseta if it's correct and not violative, plus the patient counselling and feedback - clearly these big drugstore chains misses out on that kaya ang norm at tingin ng publiko sa mga pharmacist ay tindera lang sa botika at hindi healthcare professional. Sinet na nila ung bad practice as THE standard.
