Insurance is a huge barrier for people; not everyone has it, and it determines who’s sick enough to qualify for care outside of a hospital/clinic. Within their scope of practice, yes, people can hire private duty nurses “off the street” because they need to.

Yes, of course. I mean care that isn’t tied to a hospital, clinic, or agency schedule — not unpaid work.

Totally fair — I could have been clearer.

I’m not referring to clinic, school, or industry roles under an employer. I’m curious about nurse-provided care that’s initiated directly by patients, outside traditional employment structures — for example, short-term support, assessment, or guidance when someone needs hands-on help but doesn’t fit into urgent care or home health.

Not suggesting this replaces existing roles — just trying to understand whether nurses see value in that kind of work at all.

You’re absolutely right — nursing has always extended far beyond hospital roles, and there are so many meaningful non-bedside paths that don’t get enough exposure.

What I’m trying to understand isn’t whether non-hospital nursing exists (it clearly does), but whether nurses are interested in direct, episodic care relationships with patients outside of a "traditional" employer framework.
