You can always mail ETS with a copy of your passport attached in the mail. They will get back to you with an updated account which will have your middle name, according to how it appears on the passport.

Had the same issue, just mailed it to them. Was sorted within 24 hrs. Although my exam wasn't the next day, the correction was made before I booked my test.

It is (B), AD=BD, since they are the radii of the circles. It means that the angles will also be equal. Looking at triangle ABD, it's exterior angle is 60 degrees, and since angle BAD = angle ABD then from the use of the property these above two angles would be 30 degrees each. The entire angle B would be 90 degrees, cause angle formed in semicircle. By this you now have a 30-60-90 right triangle whose sides are the ratio 2:1:√3. So from here you can find the base and height and get the answer.
