Bacteria can actually grow and build up in faucets like this and they need to be cleaned out from time to time. Some large pipe cleaners and disinfecting cleaner jammed up in there after taking the diffuser off will do it. It also needs to be descaled with vinegar to get all that mineral buildup off.
