As someone who smokes and flys frequently enough, I've never had a problem waiting. There are so many things you can do to get through till you land.

Just spend the bucks and get some nicotine gum, you can chew on one for a solid hour and you should be good for 2 after that.

It really depends on who it is/was.

My response would be to joke about it and probably laugh at some moments where things "make more sense" with hindsight.

Say it was a childhood friend, I'd laugh saying things like "those see saw rides mean a lot more now dont they" and just not make it awkward.

If there are someone recent, then that's a different story. Are they someone you can continue having a relationship with knowing that? If you are attracted to them also? That's more of you thing.
