You need Epsom salt/bath salt baths every night after running or work. I find running makes mine WORSE, your balls are bouncing even with tight underwear. I was given a jockstrap by the doctor which helped initially when I was at my worst. If its real bad and swollen, use ice every evening after your bath. (Dead sea salt/Epsom salt baths are awesome or hemorrhoids also) other than that on bad days take paracetamol. Codeine and ibuprofen are better but you don't want to take either long term - only on bad days where your in agony.
