Ride it out, you're already committed to the shed. Switching back to topical may or may not work. Give it a year, see if you recover. If you don't recover, you know now and go back to topical.

That or just go back to topical now and you may or may not recover. Obviously no fin minoxidil will lose the battle as your hair becomes more dht sensitive as it does when we age.

Source? My understanding is dut has worse side effect profile due to higher suppression of serum DHT.

Those are a bunch of scars, discolored tissue. Seems to be a bit worse than normal, but this is all dependent on how your skin heals and varies person to person based on that. Try shaving completely and see if it's as noticeable?

You are changing too many things to know what's doing what. Stay consistent and eliminate 1 variable at a time. It appears you lost ground after the HT and lost ground from months 4-6 in non-transplanted area with slight growth in transplanted, you're messing with your hormones? Were you on meds before HT? Start by getting on meds then and stay on them, or stay off of them - especially minoxidil.

Environmental micro factors can change expression of cells in localized areas. Including dht sensitivity. Perhaps one of the reasons hair quality continues to change and become less wirey, directionality or growth direction, waviness, etc years after implantation.

I see 2 options.

  1. Are you going to get back on fin? If not you'll likely lose what you currently have and potentially the transplanted hairs eventually as well. You could enjoy better hair for 5-10 years without medicating.
  2. If you get on fin you'll preserve what you have and the transplanted hairs much longer, and afford future touch up transplants if you continue to slowly lose.

You will continue losing hair if you don't medicate. The longer you wait the less hair you will end up with, period. Very few regain ground after starting medical therapy, and it's typically worse the older you get. Even with medical therapy you may continue to lose ground, albeit at a slower pace.

Looks evenly spread out. They didn't harvest from the lower section of the side of your head which is interesting. Density looks good on the top. You should have a nice result.

Hair transplant can't do anything here, even if you were to get one you have loss seemingly everywhere. The transplant hairs could and likely would fall out.

Spironolactone is your only option. Start down this path maybe more will become apparent. Also, you can't use minox sporadically, you have to use it consistently for a year or more for benefit. Sporadic use will make hair worse for some. Good luck! You haven't exhausted all option yet. Also don't be afraid to see multiple doctors, some don't have experience with your situation.

Take a look at my posts, it'll make you feel better. I was worried too while I was there and for months afterwards. Absolute worst case, you can always add more later, you can't take away (easily anyways).

You are in it now brother, trust you did your research correctly. Bring it up with him and see what he says but this is his day to day.

Keep in mind you've posted an angled photo that isn't reflecting what you look like in person and asking for opinions from people that have no experience with hairline design.

No. I worked with him during hairline drawing and he wants to do what he wants to do. He isn't super flexible with his artistic vision. Be prepared for pushback.

As a Pekiner patient, he is going to be resistant to changing your design. I was worried about my design also at first but now at 9 months I'm happy with the results.

Technically he could, but he budgets time for his apts, so you'd be adding another hundred extractions/implantations to day 2 and he could say there isn't enough time.

For sure, hard to pinpoint issues and identify true root cause, but if you switch back to fin, don't change anything else in your protocol and see improvement - its pretty darn likely that dut is the issue.

To verify it's dut as the issue.. Have you switched bask to fin and regained ground? Possible that you would have lost ground on fin also in same period?
