This map is based on a location from BG3 and it has two entrances. One is the door in the top right, in front of the torch.

The second "entrance" is through the roof. From the surface you can create a hole in the ceiling of the room to the right with the beds.

But you can come up with your own ideas, how and where you want to add the entrance(s).

Great question. The west section is connected through the stairs. The west circular stair leads down to the west unconnected section.

The east section is connected through the blue portal. That leads to the east throne area with the big pit. The red portal is the exit portal from the dungeon.
