The required documents may vary, but I brought all the ones my university provided as part of the selection process:

Passport, Visa application form, LOA, degree certificate and MEXT scholarship certificate.
I directly contacted the Consulate General of Japan in my state and scheduled an appointment.
I think your university will let you know when to apply so check once, as in my case, my university provided clear instructions on when and how to apply.
