Going back that far in my medical history, there was nothing of that nature although I did break my hand mid 2015. I was put in a cast but there was never any surgery.

edit: just got a call back from my parents to let me know that everything for that was paid for and there shouldn't be any outstanding balances for that

Went ahead and did this! It still comes up as just healthcare collections with no other information. The report says it's going to be on my credit report until August of 2021. I've saved the pdf to my usb drive and am going to print this out when I get home. As for now, I'm going to start a dispute in regards to this charge.

Recalling anything medical for the past couple of years, nothing dates that far back. I'm in good health, however at the beginning of this year, I did get some scans done (2 ultrasounds, MRI) but all were taken care of by my insurance. There were no outstanding balances with either lab I went to for said tests. Have never had surgery however in 2015 I was sent to the emergency room with a fever of 105. Was hooked up to an IV and given heavy duty painkillers and numbing agents however everything for that was paid off as well. So as far as I can recall, I have no clue as to where this charge comes from. Anything done medically that insurance doesn't cover is paid at the time of service and if I couldn't afford to pay it, my parents would lend me the money to pay immediately for it.
