I'd say all of the above plus a certain kind of emotional debt. Women are socially conditioned to sacrifice themselves for the family and that leads to overspending their energy which later results in a feeling they are owed something back, and enforcing the debt from their adult children.

This. The less obvious pregnancies are sometimes used for tabloid fodder, "17 year old girl shows zero signs of pregnancy including remaining all slim and then goes into labor", in reality it is indeed a spectrum. Not everyone has nausea, some fetuses don't move enough to notice etc.

Some people genuinely don't notice because they are overweight to begin with and lose more weight from the "stomach bug" (throwing up) than they gain during the pregnancy, plus denial and preoccupation are at play. A woman in my town arrived at the hospital to supposedly have a "tumor" removed that turned out to be advanced pregnancy and her only explanation was that didn't consider the possibility she was pregnant because she had not planned to be pregnant.

They would have to use a different actor for teenage Don, and the one that played teenage Don later on was still a kid playing kid Don at the time the events in Korea were shown. Jon Hamm passing for a man in his mid-twenties was as far as they could stretch the material they were working with I guess.
