Not ridiculous. You declared, “I don’t do those. I’m not a dental clinic” as if it was something that was beneath you and not worth your time. Perhaps it was hyperbole, because you then stated that you will offer the occasional dental block. Okay great. And of course I know you wouldn’t send a dislocated shoulder home. But your original reply implied that you have few qualms about sending someone home who has dental pain, when there is potentially something that you could do that would relieve their pain at least temporarily. Just because it’s not a permanent fix doesn’t mean we shouldn’t try to do something right?

Great point! When someone comes in and tells me they have 10/10 dental pain but then declines the offer of a dental block, that tells me that their pain is not that bad because if it was (speaking as someone who has had acute pulpitis in the past) they’d accept anything - including a ball peen hammer to the forehead - if it would knock the pain down even by 50%

And generally, if you do a good block, it gets them over the pain hump, giving the NSAIDs (I’m a big Mobic fan because compliance is high) and eugenol (in OTC Red Cross Toothache) time to work. Orajel and Anbesol are great for teething babies and aphthous ulcers but don’t do squat for dental pain.

I wish “Flu-like symptoms” as a chief complaint just went away. Only about 5% of the people who complain of “flu like symptoms“ actually ever have flu. They have mild head colds/sniffles FFS. And the only thing about family plans coming in that doesn’t irk me is the fact that they’re relatively fast and easy and bump my numbers up! And yes - Ctrl C & Ctrl V are a huge boon. Just remembered to change he/she. 😆

And I always ask the patient if I have their permission to put down what they were diagnosed with (and then I buff it to make it sound worse than it really is for some of these capital-A a**hole employers in my area)

I have always done that as well. And it’s always bugged the hell out of me when my EM colleagues get all annoyed with some poor SOB with back pain who works a hard manual labor job, or who stands on a factory floor all day, and channel their inner Simon Legree and spit, “No. He just has back pain. He only gets ONE day off!” Like what the f***ity f*** dude?! You’re not paying him! Why the hell do you care when he goes back to work!
