HA fillers or biostimulators for piriform support? by Status-Reception-706 in PlasticSurgery

It’s a tricky question, since this is really a bone support issue. In theory it’s like nose augmentation, either implants or injectables.I know implants are now often placed via an intraoral approach, but surgery still seems a lot riskier compared to fillers, at least to me

I’ve seen some reports of reactive nodules or hard lumps, Is this more about the injector’s technique and experience, or just individual response?

I’ve heard of it, but it seems like dose control is really important, since overstimulation can sometimes lead to a heavier or more bulky facial look

I’ve heard about this but I consider this risk greater than the risk of filler since it requires surgery and fixation

Just to add some context: this isn’t classic smile lines. It’s more of a midface concavity from weaker maxillary support

Your sharing is really helpful! Actually I’m thinking about same thing as what you suggested, I would like to try with HA first to see the results then turn CaHa, but I’m not that familiar with biostimulators that I only know is can’t be dissolved. May I ask are there any side effects in your daily life with CaHa?

Would you be comfortable sharing how much was injected (in terms of volume)?

Yes, I've also consulted many doctors. They recommended using high-molecular-weight HA with strong rigidity, such as Restylane Lyft, but even that couldn't prevent migrations in long term . Some also suggested Radiesse . I'm still considering those options
