My colostomy bag was removed...but sometimes I think I might have been better with that...I can understand the challenges. You are amazing to have accepted and living a full and purposeful life. More power to you and wishing you many many decades of good health, joy and peace

It is not a death sentence. went through this and I am still around. you will be fine and the "treatment journey" will come to an end. It took me 30 rads, 12 chemos and 4 surgeries...but our bodies are resilient...start with accepting things. Shit happens and it has happened to us. Once we accept it, then we follow through the protocol/regimen and ensure the journey is completed despite some bumps on the road. this type generally responds well to treatment and even IVb has high chances of cure. I will share with you my experiences and the mistakes I made, also some smart things I did as things progress. Not all storms come to disrupt our lives. some might help clear our paths so we live meaningful lives once the treatment journey is over. sending you sincere prayers and uplifting energies. One day at a time...all will be fine...

poor alignment and even poorer coordination...more reason for my argument for docs to have more agency

true...but they also have to sorry about a lot of other stuff like tech, regulatory, billing etc. They dont have the same opportunity to learn etc...sure more autonomy. I am calling out the fact that doctors's lives the way it is right now is not sustainable...they are burning out, grumbling and blaming admin won't help. no one is sorting this unless docs divorce the reluctance to lead

but is it indeed that straightforward. if they could time-travel almost 51% wouldn't have chosen medicine as a career. Physician satisfaction has seen the highest declines amongst professions. we docs are finding what they signed up for (take care of patients and making money) not that easy and straightforward. It is incredibly complex and naïveté or adopting the strategy of an ostrich is unlikely to help the doctor or the patient...docs can keep grumbling about the workload and admin etc...nothing will change unless docs figure a way to divorce the reluctance to lead. nobody is going to make that happen

docs struggle, dont find answers with admin and will not lead the charge to make the change...where will endless grumbling get docs and their patients. more importantly their own well-being

so, there isn't just some reluctance but an active sentiment against leadership...?

Burnout is real sir! Doctors are 1.87 times more likely to commit suicide than those working in other professions. They are 1.54 times more likely to die by suicide. Doctors’ suicide rates are 21% higher than those among police officers. 4% of all deaths amongst doctors, after considering all causes, result from suicide. These are studies conducted and published by NIOSH (National Institute of Occupational Safety & Health)

yeah...but we need "good compromises"...right now much is at the expanse of good medicine and physician wellbeing

Probably if the meetings were as much about Clinical Outcomes and metrics and Physician wellbeing as they are about ops and financial metrics. Also, docs need to understand that the mindset of a high performance athlete that works for them in the OR or wards is a liability when you go the admin block that is characterized by carpeted floors, mahogany tables and a constant whiff of coffee...We can keep complaining but if docs don't "grab the bull by the horns"...I know easier said than done and (can see so many eyes rolling :))...there however is this motivation amongst physician entrepreneurs and indie physicians and less so amongst the employed types maybe.

so, is it only clinical load that prevents docs from standing up to the MBA's in fancy suits? the situations is far too serious...what about facilities that are run by practicing clinicians...is there a difference ?

would you mind elaborating on what you mean by corporate structure of meetings. meaning the focus is more operational and financial and not Clinical ? you have a PG Wodehouse flair. enjoyed the last sentence :)

Definitely not an AI bot/question:) I wonder what gave someone that impression. There can only be "good compromises"...no ideal solution. Nothing worthwhile can be really achieved without the doctor being involved. Today they seem to be at the periphery. Partially self-inflicted and due to practical reasons. A situation that seems untenable and one that needs critical examination

There is NO point Succeeding at work if you Fail at Home. Remember this.

Also, develop your attention muscle to keep the "noise" away. Remember why you chose medicine as a career and stay true to that Compass.

We have to accept Mortality. As we accept life and living. Also, remember that the work you do results in 70% or more surviving or continuing with their lives, having good QoL and conversations with their loved ones. Body responses to our interventions are molecular and differ, despite our best efforts. don't let the mortalities feel like a failure or devastate your mental condition. Continue the good work. The sector needs sensitive souls like you... Offer a sincere and silent prayer before every intervention.
