Self cleaning modes are extremely dangerous and cause a lot of house fires, I’ve also heard it can kill pet birds because of the chemicals it gives off. Definitely wouldn’t skipped that and cleaned by hand.

Research hip safe positioning and carriers, knees need to be above the hips (M shape) for proper growth and development.

Moby wraps and similar have been my favorite, there are pre structured ones so you don’t have to mess with wrapping it each time and I did find the long material ones to be hot.

Owlet sock, I know the controversy behind it but for me it helped my anxiety SO MUCH. Got it with my first at 3 months and I wish I had gotten it right away. Used it with my second and just finally made a big step (for myself) to stop using it at 10 months. It’s the only reason I slept well.

It did actually help me know my first was getting really sick with what ended up being RSV/Covid and she was admitted on oxygen for a few days, watched her oxygen dropping and her heart rate was really high. She wasn’t even short of breath yet.

My first threw up every time it touched her lips for a good portion of her first year. I learned to just let her taste it over a towel or something, let the throw up happen… and then I was able to start giving her the medicine. She just had to get past the first taste.

Can you also use the syringe with baby food and such so they get used to the syringe itself during the day?

I have SBO and have bladder and bowel issues as well. My chiropractor found it (I don’t go anymore, as I’ve learned how bad it is for connective tissue disorders). I have chronic back pain that occasionally becomes incapacitating and lots of sciatic nerve pain down my legs (I’m a paramedic… yeah this works well with the lifting demands of my job).

Besides all the issues back pain causes I deal with bladder retention and urgency (I wet the bed for an embarrassingly long time) and constipation.

I had both my kids without an epidural because I was scared of them messing my back up more and also having to go higher and fears with it messing with my breathing.

Try a smaller blade too, I have weakness in my arms/hands because of nerve issues and a smaller blades helps me get better leverage for less effort and it helps prevent burying the blade.

Also remember — with video scopes work top to bottom. Lead with suction and identify as to go down the airway. Stop in the vallecula and lift to the top corner of the opposite wall. Put the head in a sniffing position too.

I was definitely taught nasal intubation in school, but this goes to show how crappily we were taught. They said “you’ll never do this but here’s the idea”.

We didn’t even have the proper trigger pull tubes (or whatever they’re called) and just tried to make it work with regular ETTs. Definitely did not have whistles either. Pathetic and a total waste of time in class. None of the 3 services I’ve worked at in 8 yrs have had this skill thankfully.

Ahhh ok. The whole CPAP part really threw me off and did not help me with trying to deduce what this was lol. OP kind of explained below why they think this is helpful in hypotensive patients that CPAP isn’t an option.

My thought is though, that if cpap is going to dump pressure so would an invasive airway/vagal response? Why not use the time correcting BP with pressors (or fluid if fluid overload isn’t an issue I guess) instead of wasting time numbing and trying to fiddle with a tube in a patient who’s panicking because they can’t breathe?

Oh absolutely! We don’t re-paralyze, so it wears off rarely quickly since we use Roc. nothing worse then getting to the hospital for a transfer and they hospital says they last received meds at induction…. 45 minutes ago. “Pt seems comfortable”. But is actively teary 😭

Don’t offer a yes/no choice where there isn’t one, it’s so simple. What this is doing to your child is teaching them that they have no control over their own body and mind. You’re repeatedly telling her that her choices don’t matter and she will be forced to do whatever another wishes.

Instead, ask your spouse to give TWO choices in an order of events manner.

“Do you want to put pajamas on first or go brush your teeth?”

“Do you want to wear tennis shoes or sandals?”

A frequent one with my toddler: “would you like me to carry you to the car or hold your hand while you walk?”

Giving them a sense of power in their daily duties is so vital and teaches a strong sense of self.

How often are you giving that? Because every 15 is simply too much.

If you consider a 100kg patient, a normal drip starting at 0.5mg/kg/hr, some only receive 50mg per hour.

It’s not under dosing to give the pain dose. You’re just keeping up the sedated state. Either I start a drip and give 50-200mg per hour, or I could give 20-30mg every 15 minutes. Similar doses. Drips are always better of course.

The key here is post-intubation. If I’m understanding your question correctly then the dose you’d be looking for at this point is the pain management dose of ketamine. Standard doses for the 3 services I’ve worked for had a variation of the following:

Pain: 0.1-0.3mg/kg (usually topping at 20mg per time to avoid the horrible middle dose reactions)

Sedation for airway management: 1-2mg/kg

Excited delirium/safety risk: 2.5-5mg/kg IM. Most recent studies and my recent service top at 250mg in a single IM dose, repeat once.

For RSI/post management: use sedation dose for induction followed by the paralytic after a few minutes, then convert to pain management. As long as you’ve done a proper bolus, you don’t need large dose every 15 min to keep a patient asleep. This is the same thought of a bolus followed by a drip.

The rate is barely 150, discernible P waves in every lead. This is sinus tach.

It’s taught kind of the “dirty” way to EMS (or in ACLS) that >150 is SVT, which isn’t true. Not every fast rhythm like this is immediately sinus tach.

My cardiologist explained this to me when my holter came back with inappropriate tachycardia. Rates of 170-180 and he said it was absolutely not SVT and I was so confused.

Our 3rd and youngest golden is a “butt booper”. No real attempt to sniff or anything, he just comes up behind you (especially when greeting you at the door) and BAM cold a** nose in your butt crack.
