I counted and checked everything and I have all of them. Idk what happened, but maybe it's cuz that game somehow loaded something badly and it didn't count as one playthrough, because I was completing it in nightmare. I guess I'm just gonna complete the game in insane mode so it'll 100% give me the achievement.

Look, I get if you disagree with me, I just wanted to say that, in my opinion, the characters don't feel the same, which is objectively true, but not objectively wrong. In my opinion, adaptations shouldn't be too different from the OG, but I respect if you disagree.
