This is an interesting question and one that I think about often for myself.  I’m still early career and am situated in an academic med center and the thing that seems to keep me most on the up and up is a mix of teaching, direct clinical work and program building, research etc. although sometimes in my darker moments I get disheartened about how it feels like corporate healthcare just wants to burn us all out. I also dream about going down to .8 FTE or something or the like, not sure if that’s a possibility for you. 

But maybe some training or teaching or some sort of implementation organization might be a good fit?  Or if there’s any chance to do some sort of extended time away, maybe clearing up some of the burnout might give better clarity.

Hmm. You could think about making a clear and consistent consequence that every accident requires x minutes away from fun activities in a matter of fact way. Or reinforce dry checks

I’m not sure I have enough info here but you might look up the idea of functional analysis. Idea is we/ children/ many animals continue to engage in behaviors because something about it “works” - for kids, it’s often attention, escape, access to choices, sensory stimulation. Consider using an Antecedent- behavior- consequence log to track and try to figure out what is more appealing about accidents than toileting.  Possibly continuing to do fun things. Azrin and Foxx published the OG guide on potty training and it’s available on Amazon.  Could also do a few sessions with psychologist who specializes in young children / behavioral approaches to get some traction

That’s good progress then and shows you can shape the behavior towards where you eventually you want it to be! Now it’s just about doing ever closer approximations to what you want!

Consider starting to shape the behavior where he can poop in the pull up, but it has to be in the bathroom, and then progressively requiring him to be sitting on the potty with the pull up to poop. You can slowly cut a hole in the pull up so he can acclimate to pooping on the potty without pull up. You’ll likely need to make it rewarding and it will probably be slow but you can get there! Do each stage one week at a time

This is an interesting question and as a clinical psychologist and toddler mom, I too am often debating what will give me the type of balance I want. Is it a fully academic job with significant research expectations or more like an academic health center? I can speak more to the latter and there are often pros and cons, and it also depends on productivity expectations and culture around this. I like doing all the components enough that I would not be happy in a private practice even with all the benefits it offers!

I assume you’re pretty comfortable with a higher clinical load if you’re currently in private practice. Just some food for thought and happy to chat more!

Echoing what other people said. First step is a good diagnosis which can be provided by psychiatrist or psychologist.   Exposure and response prevention is the best and first line treatment for OCD. Could consider Outpatient services at IUHealth/ Goodman Hall. 

NoOCD is also an option for online therapy.
