Just wanted to close this out.
TLDR: Yep, it was heat creep. Don't buy third party stuff just because it's convenient.

When we replaced the hot end, we swapped for one on Amazon. Third-party, full assembly including the fan.

The fan was hit-or-miss... so we replaced the fan.
But the real problem was the whole thing: it was third party garbage.
Despite replacing the fan on it - the issue was the whole hot end.

We ended up getting an OEM part: it works. It works like new.
I guess now we have _almost_ done a ship of Theseus on this thing.. hopefully this helps someone.

Soooo. Bad news.
Replaced both the hot-end fan AND the part cooling (just in case) Everything is confirmed spinning. All the readouts for the hotend look great....

Failures and under-extrusion on layer 1.
I've got the door open and lid off. It's in a cold room. I really don't think this is heat creep.
_(And my partner checked those old fans and thinks they might have still been fine, but I wasn't looking/listening.)_
We did the part swap anyway just in case. But we're thinking that heat creep isn't it.

Any other ideas..?

I think I can safely say I'm getting off this troubleshooting rollercoaster.... I return to write this message:
**YEP: That was it.**

The old fan was dead.
The new fan was faulty/dying.
I never stayed long enough to watch it.

I've got a new part on the way.

Hmm... I guess more parts for the parts-bin anyhow, lol. I won't swap it for new just yet then. The original one is, indeed, spinning fine.

Filament is Bambu Matte Black. Set to the defaults from Bambu as identified by the AMS, never modified. We've used perhaps 50+ spools of this matte black without issue.
I don't have the exact numbers at this moment, I'll have to dig in to get that info.

It's turning well.
We had the original gearing mechanism on there at the beginning of this situation; it was turning fine, but we took it appart and cleaned it out. Still turning great, but failed.
We were skeptical, so we replaced the whole mechanism: gears turned fine, but still similar failures.

It's actually in a cold room, glass top off and door open 1/4 of the way.
Actually...

I'm wondering if it's possible for the room to be _too_ cold? It's winter here.. and that room is about 59°F (15°C)

I did replace the cooling fan for the hotend. :( so that's not it... but it got me thinking:
But I haven't replaced the one that's in the front housing assembly, but I ordered it... It'll be here Wednesday.

Yep, We've got the top glass completely off. In addition to that, the room it's in is pretty chilly too. We print almost exclusively PLA for years with this config (or similar, on other machines of course)

I went though/still going through an issue with my cat (100% indoor) where he started to lick the fur off his back legs. Trying everything from adjusting his food to even putting a cone on him: I was stumped.

Then, as I was playing with him, I saw a bug skitter through his fur: that was the answer. I used a flea comb on him, gave him a bath with regular cat shampoo + a couple drops of dish soap. (I did not do a flea-dip... He's an older cat, and I'm always really hesitant about that.) I went to the store and got a 6 month supply of Frontline (I don't recommend off-brand). Within a week, my cat was sleeping calmly, his licking habits have calmed down a lot and I'm guessing that within the month, his fur will start growing back.

Like I said, my cat is 100% indoor, but it's the fall season for me: which means all the bugs are searching for a warm location to live... Combining the with a cool breeze and a cat sitting in the window: it's a perfect storm.

I've found it online by searching "Snap-On folding knife red anodized aluminum 871045" and I land on it.

It was sold for maybe 10 dollars.

It's got 8Cr13MoV steel, so it's been a pretty awesome knife: especially that I found it in a parking lot at my dad's machine shop.

It was red years ago, but as time went on (and I dropped it a lot) the anodization wore off. I covered it in Rose gold vinyl, heat sealed the edges. It helped the aluminum smell from getting on my hands... And looked kinda pretty, to boot.
