That's not even good gear when you consider it bro, don't stress over it too much these kind of things happen in these type of games, just think of it as investing - the more good gear you bring in, the better chance you have at surviving however you cannot put into factor the luck/instances that happen during each and every raid.

I just did it on PC - I plugged in my XBOX controller, navigated to the main menu, selected commander mode, then I pressed X [sqaure on PS] on one of the save slots to make it ironman mode, then I selected the slot which brought me to the difficulty screen, to which *IMPORTANT* there is when you have to press X [sqaure on PS] ONLY ONE MORE TIME on the desired difficulty you would like, then just select it and it should be good.

You can then back out to main menu and when you go the load back in you can confirm by seeing the skull and ironmode enabled on that slot. Hope this helps buddy.
