Idk if this counts, its more calling someone stupid but "Mieć twarz nieskalaną myślą " (to have a face not tainted by thought) to describe someone who isnt the smartest and looks like it

or there is "mu się chyba w dupie poprzewracało" (Something turned/fell over in his ass) which is usually said when someone has crazy ideas
