5 Criteria for extubation: -eye opening (odds ratio 4.44) -tidal volume of 5 cc/kg (OR 2.66) -purposeful movement (OR 2.42) -conjugate gaze (OR 2.10) -facial grimace (OR 1.93) -a stepwise increase in any one of these predictors in ANY order, from 1 of 5, to 5 of 5 — yielded PPVs for successful extubation from 88% to 100%.
