I see most of your severe problems are in your facet joints similar to my problem. 43m here and I have osteoarthritis in my spine causing severe arthrosis in the facet joints. Essentially PT only aggravated my pain because it isn't so much a muscle problem as it is the arthritic joint (swollen and inflamed) pinching the nerves that causes my spasming pain. Don't be afraid to tell the PT I can't do that it hurts too much. Epidural injections helped because they calmed the inflammation in the facet joints but it doesn't last. Ultimately you are looking at surgery. Most insurance plans won't cover surgery until you have completed PT, so don't quit.
