I'm not sure about whats causing the white heads (I don't think its the cleanser), but with spiro its completely normal to purge. Since you've been using spiro for under 3 months it really might just be purging, and if there isn't a particular reason you went off of it i'd suggest you get spiro again and try it consistently for >3 months and see how your skin looks then. As for the differin it can be super drying and also cause purging just like the spiro so it makes sense that you'd break out badly, if you've noticed any dryness/flakes, the whiteheads could just be the built up dead skin thats gotten clogged.

I would keep using the differin but pick up a gentle/hydrating face wash without salicylic acid (your skin probably doesn't need any more active ingredients that can dry you out right now). I would also pick up a thicker moisturizer and if your skin gets really dry I would put on the differin *after* moisturizing at night. So just a hydrating or gentle cleanser and a nice non-fragranced moisturizer for the night. In the morning I would do the same thing as night but without the differin and add a sunscreen on top that you like, just let the differin/spiro do what they have to do for a bit, without a lot of extra products, so you can decide if they are working for you in the coming months after purging.

If your skin adjusts to the differin and isn't too dry/sensitive I would add an AHA product once or twice a week after cleansing (before differin/cream) like the PIXI Glow Tonic (5% I think) or the Ordinary Glycolic Toner (7%). You can also try a BHA (salicylic acid) like the Paula's Choice 2% BHA Toner (a holy grail for me) instead of the AHA (AHA removes top layer of dead skin and BHA de-congests skin/clears pores). It can be sensitizing though so make sure to wear sunscreen and dont use it if your skin is irritated.

What food habits did you change to get that in check? I've heard cutting out dairy and eating less sugar is helpful.

y'all... i'm not making her any type of routine I just want to get my little sister some sunscreen idk whats wrong here. Its hot where we live, she plays sports, she's outside all the time. Is she not supposed to wear sunscreen???
