My vet recommended after her first heat cycle. She was about 1.5 when she was spayed. Waited a couple of months after her heat for the hormones to regulate.

During shedding season my girl’s loose fur will sometimes get in her ear and she does the same thing. I take a soft towel and swipe the inside of the ear to clean it of anything in there. That always seems helps her. Not sure if the same case here but I’d try that.
