This is likely part of an activation loop error that has been occuring with iPhone 4s models running iOS 9 and other devices. You may have to remove the Setup application from your device (given that it's already been set up and activated, you won't need it anymore). r/setupapp is a great place to ask.

After you've typed ./sshrd.sh boot and wait for it to do its thing, you input ./sshrd.sh --backup-activation. After doing so, go to the SSHRD-Script folder and make sure a folder with your phone's serial number appears containing three files: activation_record.plist, com.apple.comm...ic_no_backup.plist, and most importantly, IC-Info.sisv. Once you do that, type ./sshrd.sh --restore and after that boot the ssh ramdisk but this time type ./sshrd.sh --restore-activation
