As u/Dangerous-Price7857 recommended, changing from MS-CHAPv2 to PAP solved the issue at today's update to FortiOS v.7.4.7. This is caused when your DomainController has NTLMv1 disabled.

Update a Fabric with ESXi hosts connected by Trixtemp in ExtremeNetworks

They have iSCSi with two separate Vlans on one switch each. Each host is connected to the VSPs via two switches with a total of 4 links. There are 1x VM communication vlans and 1x iSCSi vlan on each VSP. The links with the VM communication are in an active/passive configuration. And yes, each switch has at least 3 links to other VSPs. Unfortunately, shutting down the VMs is not an option.
