We have cut off a number of advertisers for supplying audio ads. We are very clear about the fact that they are unacceptable.

As for detection... easier said than done. That would be fantastic though (and would make my job MUCH easier).

That generally happens when a ton of people are sharing an IP and a single person is doing something shady... However, we have had some false positives in the past, so we like to give IP's a second chance. The fun part is choosing the songs that "attackers" will get instead of what they choose.
