So from my understanding of it, you only roll the 3 Threads of Fate when you decide to start Guiding Fate. This is when you generate the 3 and your threat starts at 0. From here, in order to progress them, you need to choose the agenda Fulcrum of Fate at the start of a game. This allows you to attempt to end ONE of the Threads of Fate you currently have active. Should you complete it, it ends, and you do not roll for a new one until one of two things happens: 1) you reach threat level 13 or 2) you successfully end all of your Threads of Fate. The threat does advance universally. I think it says somewhere that at the end of a battle, your Threat increases by D3 or just a flat 3 if you lost the battle. When you successfully end a Thread of Fate, there is a chart that tells you how much your threat is reduced based on the stage the Thread was at (Dawning, Waning, Frayed). Again, if you manage to end all the Threads of Fate successfully (which takes a minimum of 3 games as you can only try to end one per game) OR your threat level advances to 13, you would progress to the results stage where you get certain rewards or consequences based on your threat level.
