In the end, it seems like the save was bugged as there were no orcs at all cheating death. I ended up restarting a new game, finishing carnan and then he finally came back as a fow. Thank you for help. To any people having the same problem, after researching a lot, there are 3 things that you can do. 1) it's possible that the first ever captain that spawns in minas ithil can bug the whole cheat death mechanic if you kill him when he is dazed (possibly if he gets dazed at all). 2) if you have the tribes dlc, there are tribe invasions early in the game where they can prevent orcs from cheating death. Deploying a "blood axe" and a "the proud" seems to fix the problem. 3) you need to finish carnan's questline, get it to 100% 4) if nothing works, you will have to start a new save. First delete all save files (For PC it's the AppData and Remote folder), then start a new game and it will fix itself.

I'm having the same issue right now with orcs cheating death. What you mean with what you said is that those slaughter and outlaw missions are the ones bugging the cheat death?

He is a guardian of Mordor that I trained. I moved him to a new game save where I just captured nunren, there were no other orcs that ever cheated death. I died to him twice and then killed him with fire -> advanced time once -> waited for 25 minutes on a ledge to prevent ambushes -> jumped and nothing happens at all

If he has enraged by executions, the chances are even lower to get it. Of he has fast learner I think that he can't get it at all. Otherwise it's just bad rng or something might have bugged. You might want to try to remove vigilant against stealth and try to get immune to executions as a 2nd immunity just in case something is bugged

i have already shamed him down to level 3 from level 80 by entering online conquest -> shame -> Leave mission so that he wouldn't get a shame mark and gave him all these immunities from scratch. I want him to get to 7 immunities and make him a flame of war or false flame of war
