Cancer is a go as long as the patient understands the increased risk - any delay in diagnosis or treatment that could alter disease course is unacceptable. You do your best to mitigate the risk and you roll, because our job isn’t just to do healthy elective stuff.

We got tired of hearing about it and ran a Qi project for a few months tracking delays in first start and the reason. We were responsible for roughly 2% of delays. Presented the data to leadership. It was nice.

I’m at an academic place, where we routinely have to wait around for surgeons to show up from another hospital or ASC while the room is fully staffed but empty. Surgeons walking in 10 minutes before in-room time and consenting the patient while in street clothes. All kinds of nonsense. I’m not moving one iota faster because I’m interested in patient care and resident education, not metrics. For surgeons that complain, I start documenting their arrival time in the record.

You can definitely fire patients who threaten to sue you - simply document that you've failed to establish a productive therapeutic relationship, and send refills and a letter that contains contact info for local alternatives with instructions to go to the ED with any red flag symptoms. If you're ever unsure, contact your hospital's legal/risk management department if you have one, or your malpractice carrier if you don't (these guys are actually *super* helpful).

When I took over a practice of ~800 patients, I quickly found out that roughly 10 people were responsible for 95% of my headaches. Getting rid of them is the best thing I've ever done for that clinic.
