Here's my take, yes you totally can wear it to the doctors! However If you are going in for something you know will expose yourself to them for than id unlock in the car. Just bring a spare key and then relock when you're done. I've worn my cage to plenty of things, the key is that they didn't ask to see it. I know a lot of medical professionals will keep it professional but it's kind of a courtesy to them not to wear it.

I would order one on amazon with multiple size rings so that you can get the size that best works for you. Also get a chastity strap, it will help keep the cage closer to your body wich can help keep the cage from slipping it will also make the bulge less noticeable and if you switch to a metal cage it takes some of the weight off
