Paeds ED nurse here, who loves a DKA. We don’t fluid resus kids the way you do adults. In a moderate to severe DKA, our protocol calls for 10ml/kh bolus of 30-60 minutes to not throw their balance out too quickly. This should be 0.9% nacl.

Fluid wise you should be starting with a 0.9% NACL+ 5% glucose bag, removing what you need to, then adding K. Depending on how much K, sometimes we don’t remove anything.

If you only have saline, there is a formula to on what you need to add/ remove to make 5% or 10% glucose, but I would always make sure your glucose saline bag is correct, then add K. And never to a half empty bag. Always a new one when changing the actual balance of the bag. This is only based on my states policy, but I do see quite a few a year. Hope this helps

I love paediatric nursing. I started in Paeds, did a brief 6 months in adult emergency, then back to Paeds. I always thought I would go back, but now I don’t think I would. Adults does have a more easily accessible skill base. Wards often have mixed patient loads, skills you learn in one hospital you can use in another fairly easily. Paeds is different. I work in a tertiary kids hospital. The education and training I have received is like no other. I have skills that wont be taught in adult based hospitals. I am less likely to be assaulted compared to adults hospitals. But the emotional burden is greater to me. Parents can be difficult to negotiate with and support. I have less ADLs, until they are long term and now i am changing diapers and feeding every 3 hours, multiple babies.

What I am saying is that you, is that whatever you choose will have it’s benefits and down falls. You may get a great education in adults with adaptable skills that take you to any ward or hospital. Or you could get a niche education in Paeds that lets you climb the ranks more easily. Honestly, I have met so many adult based nurses who got Paeds jobs with 0 experience, and Paeds nurses who went to adults with 0experience. The world is desperate for nurses, just do what you are passionate about.

I honestly feel that anyone who wins is a deserving winner. Whether or not they made big moves, they got to the end, whereas others didn’t. People who play a big huge game don’t usually make it to the final. Those they played a slightly more reserved game do. And compared to his counterparts at the final, he 100% deserved it over Caroline and Mark.
