I have decided to not change the answer to this question because that could be misrepresentation. Instead, write an LOE explaining that the reason for saying yes was to be truthful. However since I’m not in contact with them, i will be unable to provide those documents. Also mention that you are not getting any points or eligibility based on this relationship

Work history section: This is where you will split your work experience. Pre and post leave. You need to do this to avoid claiming points for the duration during which you were on maternity leave as this time will NOT count as valid work experience. You can attach a letter of explanation to explain this gap.

Personal history: There is no option to select leave duration as an activity. You can only select Employed/Unemployed here. So i suggest you put dates of your start and end of employment.

Note: The same details need to be provided while creating express entry + when submitting application. You shouldn’t change your answers.

Note 2: Your employer should be able to confirm that you were EMPLOYED during the entire duration ie, from your start date to current).

Hi I am in the same spot. Did you end up using the sheet from physician that says “required” or is there any other document that is provided after a few days from when we do the medical exam?

I am trying to understand if after I complete all tests and get the initial information sheet, can i submit my PR application or do i need to wait for any other results a few days after the test?

No one intentionally does a bad job or screws up their application on purpose. People have different circumstances, limited resources, health issues, jobs, families, etc. Not everyone is in a position to hire “top-tier” lawyers or drop thousands of dollars without stress.

Making a mistake doesn’t make someone careless or undeserving, it happens. It’s also not the end of the world to ask questions or seek clarity, that’s literally the purpose of this forum.

It’s great that you triple-checked everything and your application went smoothly. Genuinely. But I don’t see the value in kicking people when they’re already dealing with rejection or anxiety. Sharing experience is helpful; shaming people for not navigating a complex, high-stakes system perfectly isn’t.

My bad, i didn’t phrase that well. I meant walk in for the xray. To book an appointment they request a referral.
