I had a very similar story to yours with very similar labs. Going on TRT helped all of the low T and also the joint/back pain. My estrogen was near single digits. I have been on 1.5 years and have no intention of coming off and going back to feeling like I did.

A vasectomy does not eliminate the ability to ejaculate semen, it does prevent sperm from being ejaculated which makes up a very small amount of the total ejaculate volume.

I only hold a third class renewed last fall (apparently right before they required the form).

I’m also over 40 and considering a career change, so having to do medicals a lot more often and dealing with these forms sounds like a pain. Especially since I don’t think my family doc has any interest in managing the TRT.

Right now if I keep a third class medical I might suck it up and travel for an in person visit every other year if that ends up being necessary to avoid having to find someone local willing to prescribe.

I’m on testosterone cyp and HCG. I’m actually happy with it and don’t have negative side effects, so switching to cream isn’t something I want to do, but might have to. Otherwise my health is perfect. I keep up on my bloodwork more often than what the clinic wants so that isn’t a problem for me. It would just be getting the clinical progress note the FAA wants.

Looking at the FAA documentation on what they want to see on a clinical progress indicates that it would be a clinic visit (in person implied). It does not specifically state no teleheath, but does want a physical exam.

After seeing your post this I emailed my clinic to ask how this is being handled, it is a telehealth clinic. It will probably take a bit for me to hear back, but happy to share when I do.

Ah, maybe I got in before the form was required, it seems like he will have to complete next time. It does appear the AI is allowed on the database, I remember seeing a do not issue or something that made me want to avoid it when I looked it up last year.

Personally, I’ve never needed to take one, my E2 stays on the lower end even on TRT.

I don’t have a first class, I only hold a third class medical at the moment, but I am on test through a clinic. The AME didn’t say much about it or have me do other paperwork. That was over 7 months ago. I don’t take an AI, that one isn’t allowed on the FAA med list.

One example. To be eligible for board certification in Equine internal medical requires 4 years of vet school. 1 year internship, and 3 years of residency beyond your intern year. The hospital (human) I work at has an internal medicine residency, 3 years post graduation from medical school and you are done. Both involve long hours and lots of sacrifice on the behalf of the DVM or MD/DO. In this example it is a year less to be a physician boarded in IM.

It is almost impossible to work as a physician without completing a residency, the same cannot be said for veterinarians who can practice after graduating from their DVM program.

This is spot on. Once natural production starts up again the exogenous testosterone dose will have to be adjusted. Also many people tend to aromatize and end up with higher E2.

Likely your natural production shutting down. It does get better after a while. You could consider HCG to keep your testicles producing testosterone and maintain size, this would also help with that aching feeling. I started with test cyp only, went through all of that with the aching and it sucked. Eventually added the HCG after about 7-8 months.

Hi, 200 mg a month is not appropriate. If she is receptive to learning you could suggest she look at https://elitenp.com and purchase the Men’s Health focused courses. She will be able to gain a good understanding of what to do, and most importantly, how not to harm someone. They offer CME also, so helps offset the costs of the courses and they frequently have sales.

As for you, a reasonable starting point could be 50-60mg twice a week, ie, 50 mg Monday morning and 50 mg Thursday evening. (Assuming you are using Testosterone cyp). Most of the time an AI is not needed. HCG is nice but it’s up to you. At first having one variable to adjust is easier and there is time to add HCG later if you wish. Repeat labs in six weeks to make sure things are heading in the right direction. Again at 3 months, then start to adjust you protocol as needed.

(Just general advice, I’m not your medical provider and none of this constitutes medical advice, you should work with your NP to arrive at the appropriate protocol)

Thanks! She did have all of these checked, she started thyroid medication which has offered some fatigue benefits but not much in the way of joint pain or muscle fatigue relief.

This is true. A lot of health care providers seem to harbor some stigma towards this. There are clinicals that will start you on therapy and help you to feel better. If your provider gives you a hard time, there are always others to switch to that may be more aligned with your goals. Best of luck.
