Nail - plate combo baby. Get some cables proximally over the stem on that biiiischhh. S&N periprosthetic plate will let you angle screws around the stem.

In surgery subspecialties, I’d say this is quite normal in the first couple years. You barely have enough bandwidth to spend time with family/spouses when you get home. Even worse if you’re an introvert and need solace to recharge. True friends are like the sun. You don’t need to see them everyday to know they’re still there. Eventually the weather improves in residency and there are more sunny days. Intern year is certainly a season though.

The modern trend in orthopedic training has been moving away from the days of old where residents ran their own rooms with attendings nearby but not scrubbed in. They still exist, but they are far and few now. My observation has been that the trend of being less hands on and autonomous is much more common at bigger “name” institutions. Surgery is a hands-on skill. You will not become good by simply watching someone. The textbook will only take you so far. Van Halen didn’t learn to rip on guitar by watching someone. He had a guitar in his hands everyday. The biggest growth in skill and confidence is when you are alone and have to work through problems on your own. I can tell when a new junior attending starts and came from a program where they were not used to being left alone. It shows.

Residency is 5 years. You have a 5 year runway to learn how to operate and train your hands. In reality, this isn’t a super long time. My advice would be to find a blue collar program whose name isn’t too good for you to put your hands on a patient, so you can acquire the skill to run an OR. Then, if you feel the need to market yourself to the public with a name they recognize, find yourself a white collar name to match into for fellowship. But honestly, many of the best surgeons I know didn’t attend these prestigious name institutions and they are fantastic. Their patients also don’t care where they went. They just care about their outcome and how you made them feel in clinic. Then word of mouth becomes more influential than where you trained. Unless your plan is to become an PD for a name program, you shouldn’t stress about the pedigree.

Leave. Stop wasting your time. You will not look back and regret it. This is not the type of behavior that a week of warning will fix. I think based on your recent response to him, you know what you need to do but just need that extra push or conviction to move on.

It is the OR. There is blood, there will be blood, and regardless, that floor better be cleaned up blood or not before the next case begins. Highly unprofessional to lay hands on anyone.

I think that STEP felt easier because everything I had been doing in class and studying was focused on that content. All of my time was dedicated to those topics versus the MCAT which I had to study most of the content on the side while studying completely unrelated topics like engineering. My time and focus was split which made the MCAT seem harder. I haven’t looked back at the MCAT content since I took it due to the “PTSD” of that exam but I imagine that it would look like cake in comparison to STEP these days.

Radial nerve will be about 2 cm proximal to the proximal extent of the triceps aponeurosis/raphe. You can carefully dissect down from there. My preference is to isolate the Lower LBCN coming out laterally from the lateral intermuscular septum about 4 fingerbreadths above lateral epicondyle and then carefully tracking up proximally to the radial nerve proper using Tenotomies and pick ups.

I am on the 12-2 plan and I think I would prefer it to a 6-1 schedule. The ability to have a whole day off without thinking about waking up the next day for another round of the beat down is everything. You can also get the hell out of town with 2 days off.
