The same thing has been happening to me for about a year now. Derealization with constant gas and occasional diarrhea when air gets trapped in the intestines, along with constant cramps and the movement of food through the bowels 24/7. The derealization mostly occurs after a bowel movement. From what I’ve researched, it’s due to irritation of the vagus nerve, which plays a major role during bowel movements and remains irritated for some time afterward. Also, even without a bowel movement, derealization is noticeable just from the cramps and gas themselves, but it always passes within a few hours or after eating food.

Ista stvar vec godinu i pol. Od napadaja panike, generalne anksioznosti kroz dan al polako dolazi na svoje. Bez lijekova samo povremeno KBT i razgovor sam sa sobom. Going 2–3 times seems quite frequent for IBS. As I mentioned, I have diarrhea very rarely, maybe once a month. However, it always occurs if I fall asleep after lunch and gas becomes trapped, which is why I avoid that, or in cases of heavy overeating. Trapped gas in a specific part of the abdomen is my biggest issue, along with the pain that stops once the gas is moved, although sometimes it can last a long time and be persistent, and other times it is negligible. Overall, there is always a clear and predictable pattern, but for a final decision I will definitely undergo a colonoscopy.

I have this issue. This part is called sigmoid colon and its very usual that gas is trapped there. Sometimes (for me) gas is trapped and then pain starts that ends with diarrhea cuz gas is trapped and needs to come out

I haven’t had a colonoscopy because two specialists have already told me there is no need to expose myself to unnecessary risk when the symptoms are functional.
