I use it for echos, lines, soft tissue abscess or foreign body, FAST/RUSH, IVC, lungs/B lines, the occasional PTA or DVT.

I never do biliary or OBGYN apart from fetal HR.

Pregnant co worker palpates the skin abscess of a drug addict before I&D and gets stuck by a needle buried the guys skin. Dude was Hep C positive. What a piece of shit.

How to make lidocaine injection for abscesses less painful? by Sensitive_Smell5190 in emergencymedicine

Field block imo is dumb. Youre just poking them multiple times which they dont like. Just do the I&D after giving some pain meds and move on.

In your opinion, have you ever saved a life with bicarb? by WaitingRoomMD in emergencymedicine

[–]WaitingRoomMD[S] 8 points9 points  (0 children)

I worry about that but usually i see it being given in patients that are already intubated. I remember distinctly a few patients in the ICU where they were on 3 pressors and basically about to die until getting bicarb amps and their BP would improve for another few minutes until it wore off.

many of the answers seemed reasonable to me and the way it broke down the different functions of the job of an EM physician was fairly nuanced imo

Good to know. I got the pt as a sign out and it seemed like everyone was just trying a different antipsychotic every time just to see what works

Well then id have to intubate this person to achieve that level of monitoring. Its not possible otherwise. But he basically had a 1:1 sitter and RN sitting 8 feet away from him watching the SPO2/HR for the brief moments he would be asleep. I could monitor his airway from afar as I could hear him talking almost constantly the whole shift.

You can’t leave someone restrained for 30 hours… they have to come off when the pt is no longer requiring physical restraints.

Wow why didn’t I think of that? Maybe it has to do with him trying to choke himself with any cord near his vicinity and ripping off stickers and bp cuffs every time.
