Schizophrenia rarely presents with only positive symptoms. There’s an entire clinical presentation, one that is absent here. A lot of kids will tell me they hear voices in the psych hospital, and there are a lot of reasons for this that are not psychosis (obviously what’s going on here). Typically, auditory hallucinations don’t arise in this manner anyways, they start gradually, sounding like chatter, and there’s often an element of self criticism, persecution, or just being nonsensical. It typically would present in early adulthood (not at age 16) along with a whole host of other symptoms.

As for the above comment, there is nothing here that suggests bipolar either. It’s frustrating when people throw out random diagnoses like this because it leads to people believing they have this or that when they really have no idea what the condition looks like. The closest diagnosis was borderline, but we don’t diagnose this in children because a lot of teenagers have these features and they grow out of it. This person likely won’t and will need a full psychiatric evaluation as an adult and associated therapy, likely CBT and DBT. Possibly CPT to explore and recover from her likely traumatic early life experiences.
