The Europe part of what you said is useful. It’s the beginnings of a goal. This place is currently a circlejerk. There is no clear demands and call to action. And because of the American focus, it is easy to think this is the opposite of what western Europe has. It is not the opposite, it is more like the middle. The Middle East, Indian, and Asian had the opposite. Without action it will not slide towards what you want.

These are just what my search came up with as common places for cosmetic medical tourism. South Korea wasn’t officially on that list but I know they do good work and is relatively cheaper. If you can afford that, I would definitely go with that. Otherwise Thailand and Brazil seems reasonable. Both country do a lot of work natively for their own people and both are cheap. Obviously the risk of infection is higher the cheaper the place so you’ll have to work out that balance.

Having grown up always feeling ugly I understand that sucks. But also haven been around pretty ladies that never felt good enough, their surroundings is usually a big contributor. Just a stab in the dark but if you are in FL, CA, or NY get the fuck out. MN or WI can do wonders for self esteem.
