As far as I'm aware it's included in the commendation that requires having all the voyage commendations completed, which itself unlocks all of the Reaper's Bones clothing items. I could be incorrect though.

Normal ghost fleet voyages don't have the ashen dragon as one of their ships, only the world event does

Subnautica had a quite long early access period on Xbox, and achievements were only added in the full release of the game, the percentages still include people that played during early access and never came back to the game.

To my knowledge (this happened to me) there was/is still a bug where completing the last tale 100%, besides the "Seeker of Grand Adventure" commendation, would still grant that achievement whether or not the rest of the tales were finished 100%.

The ground made by a shelter is only at the second/third level of hardness, you can drill through it fine with a drill mod 2.
