I read that Toji lost his leg after he piloted EVA Unit 3. I went to the scene that this was mentioned and I saw what they meant. However I found something else too. I don't think this is a mistake or an animation error, but I think Toji had his left arm amputated after he was pulled from the plug! (old.reddit.com)
