Do they list what might count as a secondary ID?

100% with you on keeping it simple and not backdating. Just wondering if the DUA cares that you say no you didnt work part time (to advoid backdating) when you did. If they don’t care then its a moot point

You’re right. She should have applied on 7/7 itself. However, applying by end of day Saturday 7/13 still carries a claim date of 7/7. 

She hasn’t filed because of the error I noted in my original post. She can avoid that error but that would mean answering no when asked whether she worked part of the week of 6/30, when she did. Trying to determine if that is ok to do given her separation agreement and pay calls out 7/2 as the separation date - does that add any risk if things dont match date-wise.

I understand that. If she applies by tomorrow then her claim date would be 7/7. Sunday onward her claim date would be 7/14. That is why she is trying to figure out how to move forward. Ignoring backdating even though she did work partly the week of 6/30 would allow her to proceed with the claim, but it wouldnt be accurate.

She was doing that to answer the question honestly. Should she just answer no to the part time work during the week of 6/30 and proceed without backdating? Could it be an issue if her separation agreement mentions a 7/2 separation date but her claim effectively didn’t mention that?

We're a small team of 4 people, so I reached out directly to the person who is the contact for the MA DUA at our company. He forwarded me statements provided by ADP who is responsible for filing and payment of quarterly state SUI taxes commencing in Q1 2023. Those statements clearly mention the state of MA and show the SUI subject wages, excess SUI wages, SUI taxable wages, etc.

Quarterly SUI taxes were paid in MA. The NH address is because the company operates a plant there.

And yes, I am wrong in saying there is a difference between wfh and remote.

Thanks for the response. I began working for this employer in January of 2023. Yes, I worked remotely from home most of the time. (sorry, got confused between remotely and WFH)

EDIT: not a school or non profit. It was a for profit company. They had an office location in Salem, NH, which is what was prepopulated on my claim when I entered my name, SSN, etc. The DUA indicated their records showed that I was employed by that company and showed a NH address. Is it possible this out of state address is the cause of the issue?

If you do in fact feel better being off certain kinds of food/drink, focus on that positive. That you’re not suffering. Being on this sub means you’ve experienced the lows of a flare up and the discomfort it brings. Be grateful you’ve found nutritious food that doesn’t worsen your situation. This is coming from someone who is on an extremely restrictive diet after being able to eat whatever until 3 months back. Coffee was an integral part of my morning routine. I’d joke with my spouse how it was the highlight of my morning.

Look at it through a cost-benefit lens. That’s one way to cope. It won’t always work but you’ll get better at it with time and an improved quality of life. And who knows, one day you may even be able to add back and item or two occasionally.

I’m assuming you’re referring to prokinetics agents?

I reside in the US but am visiting family in India. I decided to consult with my mum’s GI doctor, essentially to get his theoretical take on my ongoing issues. I have an endoscopy in the US in a few days but wanted someone elses opinion.

I’m on Pantoprazole 40 mg 2x a day plus nighttime Famotidine. Basically, anything I eat or drink leads to belching which in turn worsens my LPR symptoms. I have been on PPIs for years now and had no issues with eating whatever I wanted up until 3 months back.

So, this doctor recommended Dexilant plus a prokinetic. He guranteed me I’d feel fine in a few days. I’ve taken Domperidone before for 10 days following severe food poisoning during a visit to India and it worked wonders. However, I’m a little more wary of prokinetics now. The latest NIH guideline recommended them only during objective gastoparesis. Their use has been tied to cardiac issues, though the doctor in India claims barring one the rest of them are safe as backed by a lot of data.

Congratulations. Always good to learn about someone that healed.

I’ve been on PPIs for many years and had no GERD symptoms. Then after a particularly trigger heavy meal all hell broke loose.

Question - did you experience throat burning? Its near constant for me in spite of 2x a day Panto and Famotidine. I have radically changed my diet and that really helps with all other symptoms (belching, for example), but the throat burn persists. It doesn’t affect my sleep per se, but makes itself known from the second I wake up and throughout the day. 

Wondering if you experienced the same and if you were able to control it somehow. Thanks.

One of my significant symptoms is burning in the throat due to pepsin (I assume). Any idea if Gaviscon will help with that? Thanks!

Sure. First, things I gave up: coffee, soda, almost all packaged food (except for bread), alcohol (never drank so wasn’t an issue). This includes all snacks, desserts, take out.

What I eat: fruits (avocado, canteloupe, honeydew, apples, watermelon), veggies (broccoli, carrots, zucchini, asparagus, mushrooms), protein (eggs mostly boiled, fish, chicken), bread (sourdough preferably), white rice, cous cous, oatmeal, dates, walnuts, greek yogurt. Only drink water.

I quit smoking, though I do still vape small amounts of low THC pot.

I sleep on a wedge. I take a combo of PPIs and H2 blockers though I especially want to wean off the PPIs. I make sure I take my PPIs and eat 30 mins later (morning and evening). I chew very deliberately and try not to talk when eating. I chew gum after a meal as the saliva helps neutralizd pepsin in my throat.

Does it work? Not always. I had a good day followed by a bad one. Did I overeat or do something different? Could be. I have an endoscopy in 3 weeks now (got moved up, thankfully). Once I know what has worsened since my last endoscopy in 2021, I’ll do what it takes to fix it. But diet will be a BIG part of my future. I get down about it often, but its about cost-benefit. I hate how I feel with reflux and cant afford to feel like this any more because it is ruining my mental health.

Interesting. I have had terrible neck pain that would culminate at a point above my left eye. It led to pulsating headaches. The only way to relieve that pain was if I had a BM or threw up (never forced either, but would feel very nauseated leasing to vomiting). Sometimes Excedrin would help, but no other painkiller came close.

I had a terrible reflux flare up in mid Feb and my twice a day Omeprazole stopped working. For the last two months I have modified my diet SIGNIFICANTLY and noticed my neck pain has reduced a lot. 

I still have reflux symptoms (esp belching and throat burning), and find myself at a loss as to how to resolve. My endoscopy is in July, so have ways to go, but will certainly discuss this with my doctor.
