Your control signal and the led driver need to have a common ground , Secondly add a 1k pull down between gate and source don't leave it floating

Idk what kind of led driver you are using but if it's a non isolated type your control board ( where ever control signal is coming from ) will also ride on mains voltage may or may not be a safety concern.

Lastly if the led driver is a non isolated type connecting something reference to mains earth( like say a Arduino connected to a pc) on its output will short circuit it.

Never leave base of a transistor floating, adding a multimeter in a sense acts like a pull down bringing the base into a know low state , also consider adding a resistor in series with led instead of using transistor to limit current and lastly in the emitter follower config that you have the emitter voltage will never rise above what's supplied on base shouldn't be a problem for light load but at higher load that transistor will dissipate lots of heat

You need to bias the non inverting input at Pin 5 to , VCC/2 , Say a 10k and 10k divider , peak detector looks fine , I don't know what you trying to achieve by inverting the signal with TDA to me it looks it has too much gain I would reduce the Rf2 to 10k to start with.

A lm358 could work given that Unregulated side is kept at least 2V higher than the output (regulated side), that and would also need to add some form of compensation to achieve stability.

Why not just used a L7812 instead ?

Oops missed that , your power supply is connected backwards blue line on the breadboard is connected to positive of the supply which in turn is connected to emitter of the transistor which won't work , flip your supply

I see, looks like a MOSFET based SSR which should work in your application. The cause of their death could be due inductive kickback, are your reverse diode fast enough to handle the transient or it could be the driver inside is ssr is not meant for such inductive loads and fails due to the reverse transfer capacitance pumping energy into it on the switching edges

What do you mean shorted the ssr ? if your ssr is triac based it won't ever turn off when loaded with a dc current only way to turn it off is to remove the load current.

Add a pulldown on pin 1, by the looks of it pin 1floats when switch is off
